Advance Registration for Clinic Attendance

Lets a provider enable Advance Registration on a clinic and lets a member of the public submit one directly from Clinic Finder search results without a full registration flow, with submissions surfaced to providers everywhere a clinic's attendance is tracked, including the Provider Dashboard.
